QUICK & EASY ONLINE APPLICATION 

Completing the online application is quick and easy.  Simply click on the link below to access the online application.  Once completed, you can submit the application via email or fax as outlined below.  Once received, we will contact you if we require additional information and will provide you with pricing.

OPTIONS FOR SUBMITTING THE APPLICATION

Option A: Email the application to adagio@sthunt.com

 1. Tab through the application and fill in the necessary fields.
 2. Select the ‘submit by email’ button on the top right hand corner of the application.  A ‘Select
     Email Client’ box will appear with three options:

              a) If you use a desktop email application such as Outlook, Outlook Express, Eudora or Mail:
                 Select ‘Desktop Email Application.’ An email with your application will automatically be sent.

              b) If you use an Internet email application such as Hotmail or Yahoo:
                  Select ‘Internet Email.’ A box will appear with instructions for first saving the application
                  and then attaching and sending it via email

           c) If you are not sure what type of email application you use:
                  Select ‘Other.’ A box will appear with instructions for first saving the application and
                  then attaching and sending it via email.

Option B: Fax your application to 519.646.5812

1. Tab through the application and fill in the necessary fields or print the blank application and
    complete it in writing.

2. Fax your completed application to 519.646.5812.

Cheques should be payable to Stevenson & Hunt Insurance Brokers Limited and sent along with your signed and completed application to:

Note: Coverage cannot be bound (effective) and a policy cannot be issued until payment in full has been received. As cheque processing times vary between financial institutions, policy issuance may be delayed by up to 10 days.
